REQUIREMENTS IN APPLYING FOR PASSPORT FOR MINORS – Here are the documents needed for Philippine passport application for minors.

Passport applications in the Philippines are done at the offices of the Department of Foreign Affairs (DFA). There are specific documents needed for submission depending if the person who will apply for a passport is an adult or a minor.

Here are the requirements in applying for passport for minors:

  • confirmed online appointment for Philippine passport application
  • duly-accomplished Passport Application Form
  • PSA Birth Certificate
  • passport or any valid government-issued ID of either parent (plus a photocopy)
  • PSA Marriage Certificate of parents if only one (1) parent is with the child during the application at a DFA office
  • school ID

In case the parents of the minor are not married, they must present an Affidavit of Support and/or Consent executed by the mother of the child. In case the Affidavit of Support and/or Consent was executed abroad, the document must be authenticated by the Philippine Embassy or consulate.

In certain cases, the DFA might request addition documents. These are usually asked when the Birth Certificate is registered late or the applicant is a dual citizen or the Birth Certificate lack a certain information, etc.
