REQUIREMENTS FOR SSS SICKNESS BENEFIT CLAIM – Here are the documents that locally-employed members of the Social Security System must secure and submit to avail the Sickness Benefit.

In times of getting sick, the members of the Social Security System may turn to a Sickness Benefit offer to help them augment their financial needs amid their absence from work. Every year, a member is entitled to up to 120 days of eligibility to the Sickness Benefit.

To make a claim, there are requirements for the SSS Sickness Benefit that the member or an authorized representative of the sick member must prepare and submit. Here are the documents needed for locally-employed members of the state institution:

  • duly-accomplished Sickness Benefit Application (SBA) Form
  • SS ID/UMID card or any valid primary ID cards/documents, as follows:
    • Driver’s License
    • Passport
    • Voter’s ID Card
    • National Bureau of Investigation (NBI) Clearance
    • Postal Identity Card
    • Any other government-issued (secondary) ID cards/documents
  • Supporting medical documents for prolonged confinement/sickness, if any:
    • Laboratory, X-ray, ECG, and other diagnostic results
    • Operating room/clinical record that will support diagnosis

Documents for Presentation to SSS:

  • If the claim is processed by the member-borrower:
    • original of any one (1) of the member’s primary ID cards/documents or two (2) secondary ID cards/documents, both with signature and at least one with photo
  • If the claim is processed by an authorized representative of the member:
    • original of any one (1) of the member’s and the authorized representative’s primary ID cards/documents or two (2) secondary ID cards/documents, both with signature and at least one with photo

The amount that the sick member-borrower will get as the SSS Sickness Benefit Allowance depends on his/her average daily credit. To apply for the claim, the requirements for the SSS Sickness Benefit can be filed at any of the nearest SSS office.

Aside from the Sickness Benefit, another benefit offered by state-run social insurance institution is the SSS Monthly Pension, one of the SSS Retirement Benefits. The other offer is the SSS Lump Sum.
