Here are the documentary requirements in applying for the OVP Burial Assistance offer of the Office of the Vice President.

Nowadays, there are government offices in the Philippines that the bereaved family of an indigent or a poor individual can turn when death takes place in the family. One of these state entities is the Office of the Vice President or the OVP.

With the office led by Vice Pres. Sara Duterte-Carpio, the OVP is offering a Burial Assistance for the family members of indigent, vulnerable, and disadvantaged individuals. It provides up to P5,000 in monetary assistence.

Here are the requirements for the OVP Burial Assistance application:

  • original copy of the filled-out Burial Application form, signed by the Authorized Representative
    • must be dated and signed within 15 calendar days, including weekends and holidays, from the date of death.
  • original copy of the Social Case Study Report or Certificate of Indigency/ Eligibility
    • Social Case Study Report should be issued by the DSWD, PSWDO, CSWDO, or MSWDO Social Worker and Certificate of Indigency/ Eligibility addressed to the OVP, or generic addressee
  • photocopy of Registered Death Certificate
    • must indicate registry number
  • photocopy of at least one (1) valid Identification Card of Deceased and Authorized Representative
    • must be valid as of the date of the interview
    • original must be presented
    • photocopy must show both front and back of the ID card
      • List of Acceptable IDs:
        • Driver’s License
        • Solo Parent ID
        • NBI Clearance / ID
        • TIN ID
        • Passport
        • UMID/GSIS/SSS ID
        • PhilHealth ID
        • Voter’s Certification
        • Police Clearance / ID
        • 4Ps ID
        • PRC ID
        • Postal ID
        • PWD ID
        • Senior Citizen ID
        • Barangay / LGU ID
        • Philippine Identification System (National ID)
