- Copy of PRC Card or IBP Card (If applicable only)
- Original Certificate of Employment [must include applicant’s position, date hired, compensation, office address, HR landline number(cell-phone number is not allowed), HR e-mail address]
- Original Personal Bank Certificate [must include account name, account number, account type, current balance, account opening date, 6 months average balance (ADB)]
- Bank Statement (original or certified true copy of bank statements/passbook for the last 3 months)
- Photocopy of ITR (Income Tax Return) or Form 2316
- Visa Fee: GRATIS (59 days below stay in Korea) / PHP2,000 (60 to 90 days stay in Korea)
For Business
- Application Form
- 1 piece of Passport size colored picture
- Original Passport
- Photocopy of Passport Bio-page (page 2)
- Original & Photocopy of valid visa/s and arrival stamps to OECD member countries for the past 5 years (If applicable only)

- Photocopy of Business Permit or Mayor’s Permit
- Original Personal Bank Certificate

For Seasonal Employment
- Korean Visa Application Form

- Medical Certificate (issued within 6 months from the date of visa application)
- General health checkups should include lab test results of AIDS, Syphilis, and Drug
- No designated hospital
- Certificate of TB (tuberculosis) Screening (issued from hospitals designated by the Korean embassy within 3months)
- Only those examined at the designated hospitals are valid
- Refer to attachment for the list of designated hospitals and Certificate of TB (tuberculosis) Screening
- NBI Clearance Certificate (E-8-1~4 visa applicants only)
- Issued within 3 months from the date of visa application
- WHO-approved certificate of completion of COVID-19 vaccination
- Pfizer, AstraZeneca, Moderna, Janssen, Covishield, Sinopharm (BIBP), Sinovac, and Covaxin
- Certificate issued by VaxCertPH or certificate issued by WHO ICV International Vaccination Certificate
For Family Visit
- Application Form

- Copy of Invitor’s(Korean/Filipino) Passport First page (or valid ID)
- Invitor’s(Korean/Filipino) Invitation letter (Designated format from Embassy or Hikorea Website/with date and signature)
- Invitor’s(Korean/Filipino) Letter of Guarantee (Designated format from Embassy or Hikorea Website/guarantee period must be 3 years from entry date/with date and signature)
- Invitor’s Letter of Pledge on Illegal stay and Unemployment (Designated format from Embassy or Hikorea Website/with date and signature)
- X-ray Result and Certificate of TB (tuberculosis) Screening (issued from hospitals designated by the Korean embassy within 3months)
- Invitor’s Korean Basic Certificate (detailed, issued within 3 months)
- Invitor’s Korean Family Certificate (detailed, issued within 3 months)
- Invitor’s Korean Marriage History (detailed, issued within 3 months)
- Korean Resident Registration Certificate (issued within 3 months)
- Korean Family Certificate under Korean’s child (detailed, issued within 3 months) (If pregnant, Pregnancy Certificate, or Maternity Handbook)
- Proof documents of Philippine family relation of Invitor & Invitee such as PSA Birth Certificate and PSA Marriage Certificate
- Documents to prove severe diseases and/or disabilities (if applicable)
- Original PSA Marriage certification or Cenomar
For Family Visit To Korean National Husband/Wife
- Application Form

- Copy of Senior citizen’s ID
