What You Need To Prepare:

  • Duly accomplished Application form for Driver’s License (ADL)
  • Notarized Affidavit
  • Damaged Driver’s/ Conductor’s License
  • Proof of Identity

Steps To Take:

  • Go to an LTO Office together with your requirements. Ask for a copy of the Application Form for Driver’s License or ADL.
  • Provide the necessary information needed in the form.
  • Get a queue number and then wait for your turn. Once called, proceed to the Evaluator Counter and have your requirements checked by the evaluator.
  • Undergo photo and signature taking.
  • Go to the cashier and pay for the fees. Receive your copy of the Official Receipt.
  • Go to the Releasing Counter and present your OR. Receive the card type license.
  • Sign on the Released Form.
