The Government Service Insurance System, more popularly called GSIS, offers several benefits for its members. Undeniably, the benefit that most members look forward to the most is the Retirement Benefit. A lot of members want to reach the age of retirement and receive monthly pension from the state-run social insurance institution.
However, there might be some circumstances that could hinder a member from reaching the age of retirement while in office. Among the factors are the abolition of an office or a position. Meanwhile, the GSIS crafted a benefit offer for the members who have been separated from service before they reached the age of retirement — the GSIS Separation Benefit.
The documents required under the Separation Benefit are different compared to the requirements requirements for GSIS Survivorship Benefit claim application, the requirements for GSIS Unemployment Benefit claim application, and the documents required in all other benefit offers.
Who are qualified to file a Separation Benefit claim? It is open for members of the Government Service Insurance System (GSIS) who have…
- been in the service for at least three years but less than 15 years, and below 60 years of age
- in the service for at least 15 years and is below 60 years of age
In case the member died during the processing of the Separation Benefit claim, the claim will be granted to the legal heirs of the deceased member. The application for claim should be made within four (4) years from the date of separation from office.

- duly-accomplished Separation Benefit Application Form
- Service Record with Leave without Pay (LWOP) Certification (indicating specific time and dates of LWOP)
- Declaration of Pendency/ Non-Pendency of Case (DPNPC) form (date administered/ notarized should be on or after receipt of notification from GSIS

- Clear photo of the 2 valid IDs (front and back)
- Clear photo of the borrower holding the 2 valid ID cards
For applications to be made through email, save the form and the photos of the other requirements as JPEG or PDF File. Email them to your GSIS handling branch and wait for the confirmation from the government agency.
