REQUIREMENTS FOR CHED MEDICAL SCHOLARSHIP APPLICATION – Here are the documents needed in applying to be a scholar of CHED.
Even if you want to take a medical course but you are not financially sufficient for it, you may still push through with the help of a scholarship. The Commission on Higher Education, more popularly called CHED, is one of the entities that offer a medical scholarship.
Who are qualified to apply for the CHED Medical Scholarship Program? The following is the eligibility criteria set by the Commission under the offer:
- a Filipino citizen
- an incoming freshmen college student
- has a GWA of at least 90% in high school or the equivalent for freshmen applicants
- not a recipient of any other scholarship or financial assistance program
- not have a failing grade in any subject
- not have been enrolled in any undergraduate or graduate program before
- in good health
- of good moral character
- not have any pending disciplinary action
There are documentary requirements for CHED Medical Scholarship application. Here are the documents that you should prepare and submit:
- two (2) recent 2×2 ID pictures
- Certified true copy of high school diploma or equivalent
- Birth certificate
- Certified true copy of Form 138 or report card
- Medical certificate from a licensed physician
- Certificate of Good Moral Character from the high school principal or guidance counselor
- Certificate of Indigency (if applicable)
- Certificate of Non-Availability of Parents’ Income Tax Return (if applicable)
