Here is an Authorization Letter Sample you may use as a guide to allow someone to process documents for you in SSS.

AUTHORIZATION LETTER SAMPLE – If you need someone to process documents for you in SSS, here’s a sample letter as a guide.

In appointing someone who can sign, collect, submit and process documents on your behalf, an authorization letter is needed. And oftentimes, in doing some processes in government-owned agencies such as the Social Security System (SSS), we are put under circumstances that disable us to do so personally.

Being a member of SSS can secure a package of benefits such during the events of death, disability, sickness, maternity, and old age. Voluntary members have the covered benefits to that of covered employees except under the EC program.

Now, in order for your representative to able to do such things for you, you must do a formal and effective letter to which a sample letter is provided below.

Check out a sample below:

June 9, 2007

To whom it may concern:

I, Rogelio Cruz, married, with residence at Brgy. 43, Pasig City, is currently an Overseas Filipino Worker based in Saudi Arabia. I do hereby authorize Maricris Cruz, my wife, of legal age, married and with residence at Brgy. 43, Pasig City, to file and process my Calamity Loan Assistance Application.

I am giving her full authorization to receive, sign, and transact any communication and requirements needed in connection with my loan application.

Any acts carried out by Ms. Maricris on my behalf shall have the same effects as acts of my own. She will bring two identification cards for verification and reference.

I am hoping for your kind consideration regarding this matter. Thank you.
