In appointing someone who can sign, collect, submit and process documents on your behalf, an authorization letter is needed. And oftentimes, in doing some processes in government-owned agencies such as the Social Security System (SSS), we are put under circumstances that disable us to do so personally.
Being a member of SSS can secure a package of benefits such during the events of death, disability, sickness, maternity, and old age. Voluntary members have the covered benefits to that of covered employees except under the EC program.
Now, in order for your representative to able to do such things for you, you must do a formal and effective letter to which a sample letter is provided below.

I am giving her full authorization to receive, sign, and transact any communication and requirements needed in connection with my loan application.
Any acts carried out by Ms. Maricris on my behalf shall have the same effects as acts of my own. She will bring two identification cards for verification and reference.
