GSIS EMPLOYEES COMPENSATION SICKNESS BENEFIT – Here are the requirements in applying for claim to the Government Service Insurance System.

One of the programs launched by the Government Service Insurance System, more popularly called the GSIS, for its members is the Employee Compensation Insurance Fund or the ECIF. It is a “purely employer-based contribution benefit”.

The ECIF is also called the State Insurance Fund. It covers sickness, disability, and death of the members of the Government Service Insurance System who are employees in the public or private sector.

In the case of sickness claim, for the eligibility, the sickness of the member must be considered an occupational disease by the state-run social insurance institution. If it is not listed, it should be proven that the risk of contracting the disease is raised by the working conditions.

With regards to the benefits, there are different forms of benefits under the GSIS Employees Compensation Program and those that the member/beneficiaries will receive is based on whether you are applying for a sickness, disability, or death claim. The general benefits include:

  1. Medical services, appliances and supplies
  2. Rehabilitation services
  3. Temporary total disability benefit
  4. Permanent total disability benefit
  5. Permanent partial disability benefit
  6. Death benefit
  7. Funeral Benefit

The list of requirements for GSIS Disability Benefit Claim is different from the documents needed in filing a claim under the GSIS Employees Compensation program.

Here are the documentary requirements set under the GSIS Employees Compensation Sickness Benefit:

  • Updated service record
  • Hospital record of confinement
  • Statement of duties and responsibilities
  • Official receipts in payment of hospital bills, professional
  • fees and medicines purchased from the drugstore

Part of the requirements under the GSIS Employees Compensation Sickness Benefit is the accomplishment of the following forms:

  • Income Benefits Claim for Payment
  • Hospitalization Claim for Payment EC
  • Attending Physician’s certification

The application for GSIS claim should be made within three (3) years since the occurence of sickness.
