REQUIREMENTS FOR PASSPORT APPLICATION – For adult applicants, here are the documents needed in applying for a Philippine passport.
One of the main requirements in traveling to other countries is presenting a passport. It is one of the primary identification documents not only in the Philippines but as well as in most countries abroad.
In PH, the passport is released by the Department of Foreign Affairs (DFA). You need to apply for it and there are documents that you must submit for the application of a passport.
For adult applicants, here are the requirements for passport application that you must prepare and bring with you to a DFA office:
- confirmed online appointment for passport application
- duly-accomplished Passport Application Form
- original and photocopy of your PSA Birth Certificate on Security Paper
- one (1) valid ID
It is important to note that married females who are using the last name of their husband must bring the original and submit a photocopy of her PSA Birth Certificate on Security Paper or Report of Marriage. If the woman will use her maiden name, there is no need to present a marriage certificate.
If the PSA Birth Certificate cannot be read or it is blurry, you may be asked by the DFA to present a Local Civil Registrat Copy.
In certain circumstances like the late registration of a birth certificate, insufficient data in the birth certificate, dual citizenship, dissolve marriage, etc., supporting documents may be required by the DFA.
