BIR Form 1902 – This form is intended for employee-applicants earning purely compensation income and non-resident citizens. This must be duly-accomplished by the applicant (on personal information) and the employer (on employment information).
Identification Documents – Either of the following which contain the name, address, and birth date of the applicant may be submitted:
- Birth Certificate
- Passport
- LTO Driver’s License
- Community Tax Certificate
Marriage Contract – For married applicants
Passport – For foreigner employees
For the TIN ID application, bring the aforementioned TIN ID requirements to the Revenue District Office (RDO) which has the jurisdiction over the area where the employer’s office (where the applicant reports) is located.
