Are you in need of some financial help due to a medical situation? In the Philippines, one of the government institutions that the people may turn to for assistance is the Department of Social Welfare and Development or more popularly called DSWD.
The DSWD has launched the Assistance to Individuals in Crisis Situations or the AICS and one of the offer under it is the DSWD Medical Cash Assistance offer. It aims to help patients who have no capacity to pay for their hospital bills, medicine purchase, treatments, medical procedure, etc.
An individual may apply for the cash aid offer to obtain assistance for the payment of the hospital bill, the purchase of medicines, or to cover the costs of therapies, surgeries, laboratory costs, and other medical procedures.
To apply for the offer, the individual who is in need of a financial aid must submit the DSWD Medical Cash Assistance requirements. The social welfare department has set different lists of documents required for application on a case-to-case basis.
Here are the general documents that the DSWD requires depending which applies on the case of the patient:
- Health Certificate or Clinical Abstract
- Summary of Discharge
- Summary of Deceased Individual
- Alagang Pinoy Tagubilin form
There are also specific DSWD Medical Cash Assistance requirements for application. Here’s a guide on the documents required:
For the payment of hospital bill
- Provisional or Ultimate Hospital Invoice / Financial Statement (with details of any outstanding dues) (including the full name and signature of the billing clerk)
- Certificate of Remaining Balance / Promissory Note
For medicine purchase
- Prescription from the physician
- The course of treatment recommendation (treatment protocol)
For medical procedures or operations
- Medical laboratory examination
- Protocol for laboratory testing
- Physician’s directive (Doctor’s order)
For therapy and other forms of treatment
- Protocol for Treatment (must indicate the date it was prescribed, complete name, license number, and signature of the doctor)
- PhilHealth Certification (must confirm exhaustion of member benefits)
- Quotation or Cost Estimate (must indicate the total payable amount, the full name and signature of the issuing officer)
- Certification from Psychiatrist or Psychologist (with specified date, complete name, license number, and the doctor’s signature)
The documents submitted to DSWD must be within the last three (3) months and they must also indicate the doctor’s name, signature, license number, and the diagnosis of the patient.
Aside from the medical cash aid offer, the social welfare department also offers a DSWD Burial Cash Assistance under the AICS program.
