In applying for a membership to PhilHealth, there are documents that you need to prepare if you are a Filipino who is working in the government or private sector in the country or a self-employed individual. Here are the documents that you will need:

  • Two valid IDs or PSA Birth Certificate
  • Two copies of the PhilHealth Member Registration Form (PMRF)
  • Two 1×1 passport-sized photos

In other cases such as in the case of foreign retirees, foreigners who are living or working in the Philippines, and Filipinos with dual citizenships, there are other sets of document required for application for a PhilHealth membership.

  • Applicant must be registered with the Philippine Retirement Authority (PRA)
  • Special Resident Retiree’s Visa (SRRV)
  • Accomplished PhilHealth Member Registration Form (PMRF) for Foreign Nationals that must be submitted to any PRA office
  • Permanent residency status
  • Valid Alien Certificate of Registration Identity Card (ACR I-Card) issued by the Bureau of Immigration
  • Valid working permit
  • Accomplished PMRF for Foreign Nationals that must be submitted to any Local Health Insurance Office
  • Accomplished PMRF
  • Certificate of Re-acquisition/Retention of Philippine Citizenship (CRPC)
  • Identification Certificate (IC) issued by the Philippine Embassy or Philippine Consulate abroad or by the Bureau of Immigration

Here are the steps on how to apply for a PhilHealth membership:

  • Step 1 — Go to a local branch of PhilHealth in your city / province.
  • Step 2 — Get two (2) copies of the PMRF from the PhilHealth branch you are visiting. Fill out the forms.
  • Step 3 — Submit the accomplished PMRF forms and your other requirements
  • Step 4 — Wait for the releasing of your PhilHealth Member Data Record (MDR) and ID.
