List of SSS Maternity Benefit Requirements for Application
SSS MATERNITY BENEFIT REQUIREMENTS – Here are the required documents in filing for a Maternity Benefit claim to the Social Security System.
One of the benefits offered by the Social Security System, more popularly called SSS, is the SSS Maternity Benefit. The said benefit offer was designed by the state-run social insurance institution to financially assist the qualified members during pregnancy, childbirth, or miscarriage.
The said benefit offer is open for female qualified members who have posted at least three (3) months of contributions within the 12-month period immediately before the semester of her childbirth or miscarriage who have informed the state-run social insurance institution ahead.
In filing for a claim, there are SSS Maternity Benefit requirements that must be prepared and submitted to the state-run social insurance agency. The list of documents needed depend on the case of pregnancy or childbirth.
For live childbirth (normal or caesarean), submit any of the following:
- Child’s Certificate of Live Birth (CLB) / Certificate of Death (COD) duly-registered with the Local Civil Registrar (LCR) with corresponding Official Receipt (OR), or Acknowledgement Receipt (AR) issued by the LCR, if the filing date of MBA/MBRA is within six (6) months from the date of delivery
- Child’s CLB / COD issued by the Philippine Statistics Authority (PSA) with corresponding OR or AR if the filing date of MBA/MBRA is beyond six (6) months from the date of delivery
- Report of child’s birth or death issued by the Philippine Embassy / Consulate General / PSA, or its equivalent document issued in a foreign country with English translation, if applicable
For still birth or fetal death, submit any of the following:
- Certificate of Fetal Death (COFD) duly-registered with the LCR with corresponding OR or AR issued by the LCR, if filing date of MBA/MBRA is within six (6) months from the date of delivery
- COFD issued by PSA with corresponding OR or AR if filing date of MBA/MBRA is beyond six (6) months from the date of delivery
- COFD issued by the Philippine Embassy/Consulate General/PSA, or its equivalent document issued in a foreign country with English translation, if applicable
For miscarriage and ectopic pregnancy cases, submit the following SSS Maternity Benefit requirements:
- Any of the following proofs of pregnancy:
- Result of pregnancy test duly-signed by a physician or municipal health officer
- Result of other diagnostic tests with corresponding report duly-signed by a physician, which may include any of the following
- Ultrasound
- Blood Pregnancy Test
- Early Pregnancy Factor
- Any of the following proofs of termination of pregnancy duly-signed by a physician:
- Pregnancy Test result
- Ultrasound result
- Histopathological Report
- Operating Room Record (ORR)
- Any of the following medical documents duly-signed by a physician:
- Medical Certificate
- Records of Consultation
- Clinical Abstract / Discharge Summary
The application process in filing for an SSS Maternity Benefit claim vary. It depends if the member is locally employed, self-employed, a voluntary member, or has been separated from work.
