GSIS BENEFIT REQUIREMENTS 2023 – Here are the full lists of documents needed in filing for a benefit claim to the Government Service Insurance System.
One of the social insurance institution giants in the Philippines is the Government Service Insurance System or GSIS. Its membership is mandated among the individuals who are working in the public sector and the government.

GSIS members who have active and updated accounts may qualify to the loans and benefits offered by the state-run social insurance agency. It is possible by regularly posting monthly contributions to your account.
Members of the GSIS may turn to the government agency in times of job loss, sickness, disability, and even death (in the case of the beneficiaries). You can check the list of the GSIS benefits below and the requirements under each.

GSIS Unemployment Benefit 2023
In times of job loss, a member of the Government Service Insurance System may file for a GSIS Unemployment Benefit 2023 claim. This is given for members who lost their job involuntarily or to an uncontrollable factor. Here are the GSIS benefit requirements under the offer:
- duly-accomplished Application Form for Unemployment Benefits Under RA 8291
- Service Record indicating last day of service/actual date of involuntary separation, and Leaves of Absences With Pay or Without Pay
- Declaration of Pendency/Non-Pendency of case prior to electronic crediting or check printing.
GSIS Disability Benefit 2023
When unwanted incidents leading to disability takes place, a GSIS member may file for a GSIS Disability Benefit 2023 claim. It can be a turn to most especially when the disability prevents the member from working making him/her lose her source of income. Here are the required documents in filing a claim:
- duly-accomplished Disability Claim Application Form
- original and duplicate copy of Report of Injury/Sickness
- duly-accomplished Form for Income Benefit Claim for Payment, Parts I and II; (1 copy, original)
- duly-accomplished Form for Hospitalization Claim for Payment, Parts I, II and III (1 copy, original)
GSIS Funeral Benefit 2023
The GSIS also offers a Funeral Benefit that aims to assist the family of the deceased member for the burial expenses. To file for a claim for GSIS Funeral Benefit 2023, there are documents that must be submitted.
For legal spouse:
- Original copy of Death Certificate of the member from the National Statistics Office (NSO)
- Two valid IDs (original to be shown, photocopy to be submitted)
- Original copy of Marriage Contract from NSO
- Original copy of NSO-certified Birth Certificate of the claimant
For claimants aside from the legal spouse:
- Original copy of Death Certificate of the member from NSO
- Original & Xerox copy of Official Receipt under the claimant’s name or if a Funeral Plan was used, a Certification from the memorial service provider that the plan was availed of
- GSIS Affidavit of Funeral Expense Form
- Birth certificate of the claimant or valid ID (issued by the government) indicating his/her date of birth
- Two valid IDs (original to be shown, photocopy to be submitted)
GSIS Survivorship Benefit 2023
The beneficiaries of the deceased GSIS member may file a claim for the GSIS Survivorship Benefit 2023. It intends to help the bereaved family of the member through financial assistance. Here are the GSIS benefit requirements that must be submitted in filing a claim:
If deceased is married
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- Marriage certificate between the deceased and his/her spouse issued by NSO
- Affidavit of Surviving spouse
- If spouse is not a GSIS member, Birth Certificate issued by NSO
- Birth Certificate/s of minor/incapacitated children issued by NSO
- If with minor/incapacitated children, Affidavit of Guardianship
- If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD
If deceased is single
- duly-accomplished GSIS Survivorship Application Form
- Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
- Death certificate of member/pensioner issued by the National Statistics Office (NSO)
- Last Day of Actual Service
- Two (2) valid IDs of payees
- If with minor/incapacitated children, Affidavit of Guardianship
- If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD; and
- If guardian is not a GSIS member, Birth Certificate issued by NSO
GSIS Retirement Benefit 2023
When a GSIS member reaches the retirement age, he/she may file a claim for the GSIS Retirement Benefit 2023 that aims to provide a monthly pension or a lump sum to the retiring member. The GSIS retirement benefit requirements depend on the chosen retirement program of the member.
The benefit claim may be filed to the GSIS over-the-counter or through the GWAPS KIOSK, EGSISMO, or GSIS online.
