- Affidavit of funeral expenses
- Report of Death in case the demise is work-related
- Filer’s photo and valid IDs
Upon the presenting of the documents to the Social Security System, they must be in original and certified true copies. Aside from the Funeral Benefit, the state-run social insurance institution has another benefit offer under the same case — the Death Benefit.
