REQUIREMENTS IN APPLYING FOR PHILHEALTH – Below is a list of the documents needed in applying for membership to the Philippine Health Insurance Corp.
If your monthly budget cannot anymore cater getting a health insurance that you can turn to for a full coverage during medical instances most especially during emergencies, another thing you can do is to get yourself to be a member of PhilHealth.
A membership to PhilHealth will not only benefit you but as well as your primary beneficiaries – your parents if you are on a single status and your spouse and children if you are married.
The membership of PhilHealth does not only include emergency cases but as well as non-emergency ones like child birth, surgery, and other medical-related situations.
However, to be eligible to the benefits offered by the health insurance agency, you must first prepare the requirements in applying for PhilHealth membership so you can be a member. Prepare the following:
- two (2) copies of the PhilHealth Member Registration Form (PMRF)
- two (2) latest 1×1 ID pictures
- two (2) valid IDs or a photocopy of your PSA-issued Birth Certificate
- Barangay Certificate
To apply for the membership, simply bring your documents to a branch of PhilHealth in your city or municipality. If you are a members of PhilHealth, you just have to present your PhilHealth ID whenever you will claim for an assistance like in the case of hospitalization.
