Step 5 – Generate your SS Number. Click the “Generate SS Number” and wait for your SS Number to be displayed on the screen and you will be shown an option to print your ePersonal Record Form and and SS Number Slip.
Step 6 – Print the forms. Print your SS Number Slip, ePersonal Record Form, and the SS Number Application Confirmation which were sent to you by SSS to the email account you registered.
Step 7 – Go to SSS. Proceed to the nearest branch of SSS and submit the forms you printed.
