HOW TO APPLY FOR PHILHEALTH MEMBERSHIP ONLINE – Here is a guide in the online application process for you to be a member of the Philippine Health Insurance Corporation.

In the Philippines, the state-run health insurance agency is the Philippine Health Insurance Corporation or more commonly called PhilHealth. Millions of Filipinos are members of the said health insurance corporation.

Many Pinoys who are members of the health insurance corporation turn to PhilHealth in times of need for medical assistance due to unexpected hospitalization, birthing, operation, and several other health procedures.

To apply for membership, you can go to a branch of PhilHealth personally or you may also do it online. Below, you can find the steps on how to apply for PhilHealth membership online and the requirements in applying.

Steps on How To Apply for PhilHealth Membership Online

Step 1 – Obtain an online copy of the PhilHealth Member Registration Form or PMRF. Fill in the form with the necessary information. To get an online copy of the form, you may visit – https://www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf.

Step 2 – Save a copy of your PhilHealth Member Registration Form or PMRF in PDF or JPEG form.

Step 3 – Prepare a copy of your valid ID in JPEG form.

Step 4 – Submit your PMRF and your ID to [email protected]. With regards to the email subject format, follow this –Register<space>Name<space>City/Province, Region.

Step 5 – Wait for your PhilHealth Identification Number or PIN which will be sent to you by PhilHealth.
