To apply for the offer, there are documents that you need to prepare and submit to the Department of Social Welfare and Development. Here is a guide on the requirements:
- Death Certificate which may be released by the hospital or the doctor who pronounced the death of the deceased. The social welfare department accepts a certificate from an Imam in the case of Muslim.
- Funeral Contract which outlines the important details about the burial. It includes the date, time, and place of wake and interment and the costs. An authorized staff of the the funeral home and the family of the deceased must sign this document.
- Statement of Account from the funeral home
- Barangay Certificate which must certify the residency and the indigency of the deceased person. In case the person died outside his/her home town or province, a transfer permit from the previous barangay where the deceased was living may be submitted.
- Promissory Note if the case that the bereaved family is facing is having a balance or remaining expenses after the DSWD Death and Burial Cash Assistance have been paid out.
- Social Case Study Report (SCSR) prepared by a trained social worker
There are a few steps in applying for the cash assistance offer to the Department of Social Welfare and Development. Here is a guide on the application process:
