  • Step 2 — Click/Tap “Submit Maternity Benefit Application” under the E-services tab.
  • Step 3 — Read the important reminders, then click/tap “Proceed” to continue.
  • Step 4 — Provide all the required information then click/tap “Proceed” for the validation of the information that you have submitted. Afterwards, confirm your eligibility to the Maternity Benefit offer.
  • Step 5 — Provide the additional information like the Type of Claim and the Disbursement Account. Tap/Click “Proceed”.
  • Step 6 — Upload all the required supporting documents needed to file your SSS Maternity Benefits claim. Make sure that all the documents are clear and readable. Also, the file sizes must not exceed 2MB. Afterwards, click/tap “I Certify and Submit” to proceed.
