REQUIREMENTS FOR GSIS SURVIVORSHIP BENEFIT CLAIM – Here are the documents required in filing for a Survivorship Benefit claim to the Government Service Insurance System.

A membership to the Government Service Insurance System, more popularly called GSIS, does not only benefit the members but as well as their families. Many of the offers of the state-run social insurance giant can extend to the immediate family of the members.

The Government Service Insurance System has several offers. It has a set of the GSIS loans and benefit offers that attend to circumstances such as calamity, unemployment disability, etc. Members who maintain an updated account may qualify to the offers.

One of the benefit offers is the GSIS Survivorship Benefit. This benefit offer provides the members the peace of mind that they have something to leave behind for their families in the event of death.

In the case of the family of GSIS members, the amount that they can get under the GSIS Survivorship Benefit offer can help them financially after the passing of the member most especially if the deceased is the sole earner in the family.

The legal spouse and the children of the deceased member are considered the primary beneficiaries who are prioritized in the filing of a claim for the GSIS Survivorship Benefits. In the absence of the primary beneficiaries, the parents who are the secondary beneficiaries may claim the benefit offer.

In filing for a claim, there are requirements for GSIS Survivorship Benefit that must be submitted. The list depends on certain factors such as the following:

  • GSIS Survivorship Benefit Application Form
  • Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
  • PSA Death Certificate of member/pensioner
  • Last Day of Actual Service; and
  • Two (2) valid IDs of payees
  • PSA Marriage Certificate between the deceased and his/her spouse
  • PSA Birth Certificate of spouse if spouse is not a GSIS member
  • PSA Birth Certificate/s of minor/incapacitated children
  • If with minor/incapacitated children, Affidavit of Guardianship
  • If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD
  • GSIS Survivorship Benefit Application Form
  • Certification by agency/employer as to whether the deceased has a pending administrative/criminal case
  • PSA Death Certificate of member/pensioner
  • Last Day of Actual Service; and
  • Two (2) valid IDs of payees
  • If with minor/incapacitated children, Affidavit of Guardianship
  • If the legal guardian is not the natural parent, the affidavit should be supported by a Certification from the Barangay and DSWD; and
