PWD ID REQUIREMENTS – Here are the documents required in applying for an ID issued to persons with disabilities.
In the Philippines, a lot of people are benefitting from the privileges and benefits brought by the Person-with-Disability identification document or more popularly called the PWD ID. Issued by the Persons with Disability Affairs Office or the city/municipal Social Welfare and Development Office, its crafting is in the pursuit to help lessen the financial burden of the individuals who are dealing with disabilities.
The issuer of the PWD ID may vary among cities and municipalities across the nation. It is a renewable ID that can be renewed every five (5) years. There is a minimal cost in renewing a PWD ID.
A renewal of a PWD maintains the access and the eligibility of the individual to the benefits and privileges of the card. It can be done personally or online at the National Council on Disability Affairs.
For the first time applicants for the said identification document, it is free of charge. In applying for the PWD ID, there are requirements that must be prepared and submitted to the Persons with Disability Affairs Office or the city/municipal Social Welfare and Development Office in your area, whichever issues the said type of ID.
Here are the PWD ID requirements for application:
- duly-accomplished PWD ID Application Form which is available from any of the following:
- Department of Social Welfare and Development (DSWD) offices
- Office of the Mayor
- National Council on Disability Affairs (NCDA) or its regional counterpart
- DOH Online Registration System
- Participating organizations with memorandums of agreement with the DOH
- PWD ID application form from the DOH website
- Office of the Barangay Captain
- any valid government-issued ID
- two (2) recent 1×1 ID photos with name and signature or thumb marks at the back
- latest school assessment/medical certificate/or disability certificate confirming disability condition from government facility or licensed private clinic or medical professional
