  • Serious misconduct
  • Willful disobedience to lawful orders

  • Fraud or willful breach of trust/loss of confidence

There are a few documents required in applying for the DOLE Certification for the SSS Unemployment Benefit filing of claim. Here are the requirements in applying for the said document:

  • one valid ID
  • any of the following documents:
    • copy of Notice of Termination of Employment from the employer
    • notarized Affidavit of Termination of Employment
  • UMID card enrolled as ATM; or
  • UBP Bank Card or bank statement/certificate indicating the name of the SSS member, savings account number, bank branch, and bank address

Once you have complied the required documents, you can file your application for certification at any of the following:

  • DOLE Field or Provincial Office where the company or employer is based for employees and household workers in the Philippines
  • POLO where the employer operates or the DOLE Field or Provincial Office where the overseas Filipino worker (OFW) who lost the job resides for OFWs
