List of Requirements for DSWD 4Ps Program Application Process
Here are the requirements in applying for the 4Ps Program of the Department of Social Welfare and Development (DSWD).
Millions of Filipinos are recipients of the DSWD 4Ps Program or the Pantawid Pamilyang Pilipino Program in the Philippines. Every month, they receive cash assistance from the government agency as aid in sending the children in their family to school.
The cash benefits under the program provides assistance for childcare, pre-school, and high school students. The families have been evaluated by the government agency and re-evaluations are done from time to time.
In applying for the Pantawid Pamilyang Pilipino Program or more popularly called 4Ps, there are documents that you must submit to the Department of Social Welfare and Development. Here are the requirements for the DSWD 4Ps program application process:
- copies of birth certificates of all family members
- copies of school IDs or first grading period school report cards of all children in the family who attended school
- barangay certificate / certificate from the municipality
- valid IDs of the family member attending the community assembly
- 2 1×1 picture with white background
- copy of health records of 0-5 years old member of the family from the health center where they have their check-ups
Aside from the cash benefits for the children in families qualified for the Pantawid Pamilyang Pilipino Program (4Ps) of the government agency, the DSWD program also provides vaccination programs for kids who are aged 6 to 14.
Furthermore, the DSWD also facilitates development workshops for families who are recipients of 4Ps. The program focuses on medical and educational assistances for low-income families. It has set conditions with the recipients to ensure the success the goals behind the anti-poverty strategy which is the biggest of its kind in the country.
