REQUIREMENTS FOR PCSO MEDICAL ASSISTANCE – Here are the documents needed in applying for a medical aid to the Philippine Charity Sweepstakes Office (PCSO).
The Philippine Charity Sweepstakes Office (PCSO) is most popular for the lotto games it operates. It runs several minor and major lotto games and several Filipinos became multi-millionaires following a lotto win.

- duly-accomplished and signed PCSO IMAP application form

- Endorsement from the Medical Social Services of the health facility (for charity or service patients)
- Police Report (for medico-legal cases)
