REQUIREMENTS FOR DSWD CASH AID – Here are the documents needed in applying for DSWD Medical Assistance if you need over P10,000.00.

The Department of Social Welfare and Development (DSWD) has several cash assistance offers to Filipinos in crisis situations. One of these is the medical assistance.

You may apply for DSWD cash aid if you have been diagnosed with a medical condition and is in need of hospitalization or surgery but does not have the capacity to pay for the medical expenses. Requirements for DSWD Cash Aid for Medical Assistance:

• Medical Certificate or Clinical Abstact that indicates date when the document was given and the full name, license number and signature of doctor. It should have been given within the last three (3) months.

• Discharge Summary indicating the date it was given and the full name, license number and signature of the doctor. It should have been given within the last three (3) months.

• Certificate of Confinement indicating the date it was given and the full name, license number and signature of the doctor. It should have been given within the last three (3) months.

• Death Summary indicating the date it was given and the full name, license number and signature of the doctor. It should have been given within the last three (3) months.

• Alagang Pinoy Tagubilin Form that indicates the diagnosis, the date it was given and the full name, license number and signature of the doctor. It should have been given within the last three (3) months.
