PhilHealth Requirements Needed for Submission for Membership / To Become Member
PHILHEALTH REQUIREMENTS MEMBER / MEMBERSHIP – Here is the list of requirements that must be submitted to become a member of the Philippine Health Insurance Corporation (PhilHealth).
In the Philippines, one excellent and wise move that you can do for your family is to get you and them to become a member of the Philippine Health Insurance Corporation or more commonly known as PhilHealth.
The PhilHealth is a government-owned corporation that is set to ensure a sustainable health insurance program for the Filipino people. It is suggested that every family should have a member of the state-owned corporation.

In case you want to apply for membership in any PhilHealth office, you may start with gathering the PhilHealth requirements for membership or to become a member of the corporation.
Here is a list of the PhilHealth Requirements that you must prepare:
- Get a copy of the PhilHealth Member Registration Form or PMRF and secure it with true and accurate information.
PhilHealth Member Registration Form (PMRF) - Two (2) Valid IDs
- Two (2) 2×2 ID Picture (Latest Photo)
- Photocopy of Receipt or documents proving payments to PhilHealth (For not SSS pensioners; GSIS pensioners; AFP, PNP, BFP, and BJMP members; and GSIS Disability pensioner or SSS disability pensioner.)
For SSS Pensioners, submit a photocopy of your Death, Disability and Retirement (DDR) indicating the date when you retired or started having a pension.
For GSIS Pensioners, submit a photocopy of a certification indicating the date when you retired or started having a pension or a photocopy of your Service Record duly signed by your employer of not less below 120 months.
GSIS pensioners may also choose to submit a photocopy of his or her retirement voucher from GSIS.
For AFP, PNP, BFP, and BJMP officers, submit a photocopy of your order indicating the date when you retired. You may also submit a photocopy of your Letter for Approval of Retirement by the GSIS.
For GSIS and SSS disability pensioners, submit a photocopy of your Death, Disability and Retirement (DDR) indicating the date when you retired or started having a pension or a photocopy of your Disability Pensioner Certification by the GSIS or SSS.
You may bring and submit the aforementioned PhilHealth requirements to the nearest PhilHealth office for the membership application or become a member of the state-owned health insurance program.
