DSWD REQUIREMENTS FOR CASH ASSISTANCE – Here are the documents required in applying for Medical, Burial, Food, and Transportation aid to the social welfare department.

One of the longest-standing government institutions that are in the pursuit of helping the Filipino people most especially the poor for decades now is the DSWD. Officially called the Department of Social Welfare and Development, it has several cash aid offers.

The DSWD has helped countless Filipinos in times of natural disasters, crisis, and other challenging situations. It has launched several programs that benefit countless people. One of these programs is the Assistance to Individuals in Crisis Situation.

More popularly called AICS, the program offers several kinds of cash assistance. It aims to help people in times of medical emergencies, death of indigents, and transportation and food needs among others.

DSWD Requirements for Cash Assistance

In availing the different cash aid offers, there are specific lists of DSWD requirements for cash assistance that an applicant must prepare and submit to the social welfare department.

Here is a guide on the DSWD requirements for cash assistance offers:

Medical Assistance. Indigents may turn to the social welfare department in times of need for cash assistance for medicine purchase, hospital bill payment, costs of surgical procedures, etc. Here are the documents required by the government agency based on which applies:

  • Medical Certificate form a Licensed Physician
  • a Discharge Summary Form the Hospital
  • Death Summary if the patient passed away
  • Final Hospital Bill
  • Statement of Account from the hospital
  • Certificate of Balance from the hospital
  • Doctor’s Prescription
  • Treatment protocol form the attending Physician

In the case of basic medical procedures, here are the additional requirements for application:

  • Laboratory Protocol
  • Doctor Order
  • Laboratory Request

In the case of therapy and other treatments, here are the other DSWD requirements for cash assistance:

  • Treatment Protocol
  • PhilHealth Certificate
  • Quotation including the full amount to be paid, must bear signature of the issuing officer
  • Psychiatrist Certification indicating the doctor’s signature and the license number

Burial Assistance. The AICS also offers a Burial Aid that families of indigents may apply for provided that the following requirements, whichever is applicable, is submitted to the government agency:

  • Officially authenticated copy of the death certificate (certified true copy)
  • Agreement for funeral services
  • Verification of barangay indigency or residency for the claimant
  • Valid identification of the claimant (valid ID)

Educational Assistance. Filipino students who belong in underprivilege situations may also apply for the Educational Assistance. Here are the documents required by the government agency:

  • Application form or Enrollment Certificate
  • Officially verified student identification card
  • Proof of barangay indigency or residency for the claimant
  • Valid identification of the claimant

Food Assistance. For individuals who are taking care of their sick relatives and the beneficiaries of the Transportation Assistance, a food assistance may be applied for and the following documents may be required:

  • valid ID
  • any of the following:
    • Referral Letter
    • Medical Certificate

Transportation Assistance. The DSWD also extends a Transportation Assistance for different needs like the need to appear for a court hearing or an indigent needs to go home to his/her home province. Here are the DSWD requirements for cash assistance application:

  • valid ID
  • any of the following:
    • Police Blotter/Certification (for victims of theft, illegal recruitment, etc.)
    • Death Certificate
    • Medical Certificate (fit to travel)
    • Court Order/Subpoena/Referral
